Qualifications

Responsibilities

  • Effectively coordinate the scheduling of home care personal care assignments, including adjusting schedules based on changes and communicating with all stakeholders.
  • Ensure required authorizations in the EMR for approved personal care.
  • Ensure all care logs and timesheets are received from vendors and in-home staff visits.
  • Implement procedures to ensure a successful experience for providers, staff, and participants, such as reminder calls and sending materials ahead of time.
  • Maintain current and accurate records of all scheduled home care personal care assignments, including both the scheduled times and outcomes.
  • Be willing and able to work a varied schedule that may include evenings, nights, weekends, and overtime.
  • Complete all required documentation in a timely and accurate manner.
  • Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
  • Follow company policies and procedures and participate in any required quality improvement activities, staff training, and meetings.
  • Communicate regularly with supervisors and staff regarding workload and priorities.
  • Ensure timely completion of all mandated trainings and education.
  • Ensure timely completion of all mandated occupational health screenings as needed.
  • Exercise flexibility in performing assignments as business needs evolve.
  • Perform other duties as assigned.
