Qualifications

• Active IL Nursing License required.
• Current CPR certification required.
• Experience with G-tube and Trach care preferred.

Responsibilities

• Provide 1-on-1 patient care.
• Follow physician care plans and document progress.
• Monitor patient condition and chart changes.
• Assist patients with daily activities.
• Take and record vitals.
• Provide caregiver relief as needed.
