Job summary

Qualifications

• Active Registered Nurse (RN) license in the state of Virginia.
• Previous nursing experience required; home care experience preferred.
• Strong clinical assessment and critical thinking skills.
• Familiarity with electronic medical records and medical equipment.
• Excellent communication and interpersonal skills.
• Ability to build rapport with patients and families.
• Ability to work independently and manage time effectively in a home care setting.

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Assess patient conditions and develop individualized care plans.
• Monitor and document changes in patient health, symptoms, and behavior.
• Administer medications and treatments as prescribed.
• Collaborate with physicians and other healthcare professionals to coordinate care.
• Educate patients and families on health maintenance and disease prevention.
• Facilitate referrals to specialists and community resources as needed.
• Maintain accurate and timely medical records.
• Offer emotional support and guidance to patients and their families.
