Addus HealthCare in Birmingham, AL is seeking an experienced Registered Nurse to provide in-home care. You will enable patients to remain independent at home, perform supervisory visits, and initiate starts of care with families and clinicians.

Qualifications

• Graduation from an accredited school of nursing.
• Current RN licensure.
• Must be able to travel locally by own vehicle with a valid driver’s license and auto insurance.

Responsibilities

• Enable patients to stay independent while they remain in their own homes.
• Perform routine supervisory visits to evaluate the plan of care and supervise delivery of excellent client service.
• Complete initial Starts of Care.
• Present educational in-service trainings.
• Communicate with Case Managers and other members of the care team
