*Registered Nurse (Home Health) – $90K-$110K Annual Salary!*

A leading *home health* organization is currently seeking a skilled and compassionate *Registered Nurse* to provide in-home patient care. In this role, you will manage all aspects of patient care, ensuring high-quality clinical services within a supportive and collaborative environment. This is a *full-time*, Monday-to-Friday position with competitive salaries—higher pay is available for those with Certified Home Health Agency (*CHHA*) experience. This a full-time direct hire opportunity offering competitive salary, *paid medical insurance*, and *PTO*.

*Key Responsibilities:*
* Provide skilled nursing care in patients’ homes, ensuring individualized and patient-centered treatment.
* Conduct assessments, develop care plans, and monitor patient progress.
* Collaborate with interdisciplinary teams to coordinate and optimize patient care.
* Educate patients and families on managing chronic conditions, medication adherence, and home safety.
* Maintain accurate documentation and ensure compliance with home health regulations.

*Requirements:*
* Active Registered Nurse (RN) license in NY.
* Experience in home health or a Certified Home Health Agency (CHHA) preferred.
* Strong clinical assessment and critical thinking skills.
* Excellent communication and patient education abilities.
