Job summary

Trinity Health At Home's Mount Carmel Home Care in Columbus seeks a Physical Therapy Assistant (PTA) to join our home-based care team under supervision of a physical therapist. The PTA will provide one-to-one PT services in patients' homes and help with devices, education, and exercise programs.

The role emphasizes patient-centered care, collaboration with a supportive team, and opportunities for professional growth. A $5,000 sign-on bonus is offered for qualified candidates; driving required.

Qualifications

• Two-year PTA program approved by the APTA is required.
• Minimum of six months experience under a qualified Physical Therapist.
• Home health experience preferred.
• Must have current driver’s license and reliable transportation to work sites.

Responsibilities

• Provide one-to-one care with patients in their homes.
• Follow a written plan of care and supervise devices as directed.
• Instruct patients and families on use and care of therapeutic devices.
• Collaborate with therapists and colleagues in a patient-centered environment.
• Maintain professional standards under supervision.
