Metro Physical & Aquatic Therapy is seeking a motivated full-time Physical Therapist to provide in-home care in New York. All levels of experience are welcome and training will be provided to new graduates.

The anticipated base salary range is $90,000.00-$105,000.00 with incentives and comprehensive benefits. Flexible work-life balance and autonomy across seven days are offered, with comfort in working in patients' homes.

Qualifications

• Physical Therapist Program Graduate
• Licensed Physical Therapist in NY or eligible
• Home health experience preferred but not required
• Ability to travel to patients' homes within geographic territory

Responsibilities

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards

The anticipated base salary range for this position is $90,000.00-$105,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Additionally, this position is eligible for incentive compensation. The Company's incentive compensation plan is subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
