Job summary
Graceful Touch Home Care LLC in South Carolina is seeking a dependable PRN Registered Nurse to conduct in-home assessments and follow-up visits for our clients.
The role requires SC RN license, CPR/BLS, a valid driver’s license and reliable transportation, strong assessment and communication skills, and the ability to travel within assigned areas. Home care experience is preferred, with visits typically planned as short, in-home assessments.

- Conduct in-home health assessments and evaluate client needs.
- Monitor vital signs and changes in health status.
- Develop and update individualized care plans.
- Complete required documentation accurately and promptly.
- Communicate concerns or changes to the agency.
- Provide follow-up and supervisory visits as assigned.
- PRN/as-needed schedule
- Home visits required
- Visits typically take approximately 30 minutes
- Minimal paperwork/documentation
- Independent travel within assigned service areas required
- Current South Carolina RN license
- CPR/BLS certification
- Valid driver's license and reliable transportation
- Strong assessment, communication, and time-management skills
- Home care experience preferred
