Job summary

Advanced Technologies Home Care is seeking an LPN/RN to promote and restore patients' health by completing the nursing process, collaborating with physicians and a multidisciplinary team, and providing physical and psychological support to patients and families. Reports to the Director of Nursing.

The role requires an unrestricted Missouri nursing license, CPR certification, and at least 2 years of direct patient contact in SNF or in-home care.

Qualifications

• LPN must be 21 years of age with a HS diploma and a Certificate of Practical Nursing and/or Associate Degree, and have a current unrestricted license to work in Missouri and CPR certified.
• Minimum of 2 years of direct patient contact with a Skilled Nursing Facility or an In-Home Care Agency.
• Clinical Skills, Bedside Manner, Infection Control, Nursing Skills, Physiological Knowledge, Administering Medication, Medical Teamwork, Multi-tasking, Listening, Verbal Communication, Health Promotion and Maintenance

Responsibilities

• Provide skilled nursing interventions in the treatment of the patient’s illness, rehabilitative needs, and preventive care.
• Utilize a skilled approach in the provision of problem-specific therapeutic interventions, teachings, and training activities in accordance with the plan of care.
• Develops sales by making initial presentations; explaining products and services; and by performing community outreach. Provide education to clients, hospital staff, physicians, and community stakeholders on Home Care Services and the admission process.
• Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members.
• Confer with the RN supervisor regarding needed changes in the Plan of Treatment/Care.
• Provide hands-on care and patient education utilizing At Home Care Policy.
• Treat wounds, change wound dressings, give injections, administer enemas
• Collect specimens from patients
• Administers medications and sets up medication dispensers.
• Measures and records vital signs
• Reports and observes changes in patient conditions to the Director of Nursing
