GBMC HealthCare is seeking a Hospice Home Care Nurse to assess patient and family needs in a home care setting and provide skilled nursing support. The Hospice Home Care Nurse reports to the Team Manager. He/she is responsible for identifying patient/family/facility care needs, and for providing nursing and supportive care for the terminally ill patient and their families.
Education: Diploma, ADN and/or BSN from an accredited institution
Experience: Two years of nursing experience, or equivalent combination or education and experience.
Pay Range $66,942.34 - $102,872.41 Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
