Pay Rate:$32.00 – $34.00 per hour

Weekly Commitment:Approx. 2 hours per week (1–2 visits)
Service Area:Athens (Clarke County) & DeKalb County, GA(Must be willing to travel between these areas as needed)
Conduct scheduled home health visits (typically 1–2 visits per week).
Perform supportive supervisory, quality assurance, and reassessment visits (under the direction and delegation of the Registered Nurse).
Provide skilled nursing care and routine home care LPN visits as required by the patient’s plan of care (e.g., wound care, medication reconciliation, vital signs monitoring).
Document clinical findings, visit notes, and patient progress accurately and timely in compliance with agency and state regulations.
Coordinate closely with the clinical supervisor (RN) regarding any changes in patient status.
License:Active, unrestricted Licensed Practical Nurse (LPN) license in the State of Georgia (or multi-state Compact license).
Experience:Minimum of 1 year of clinical nursing experience (prior home health, private duty, or community health experience is highly preferred).
Travel:Reliable transportation, valid Georgia driver’s license, and active auto insurance (required to travel to patient homes in Athens/Clarke and DeKalb counties).
Skills:Strong clinical judgment, excellent communication skills, and the ability to work independently.
Compliance:Ability to pass a background check, drug screen, and provide up-to-date TB screening results.
