Job summary

Home Caregivers Partnership seeks a Licensed Practical Nurse to provide skilled nursing services in patients' homes in the Roosevelt, UT area under physician orders and the plan of care. The LPN will work under the supervision of RNs and the Clinical Manager, delivering treatments and teaching self-care techniques to patients and families.

Responsibilities include preparing notes, assisting with procedures, maintaining records, and ensuring adherence to agency policies.

Qualifications

• Graduate of an approved school of practical nursing and licensed in the state.
• Minimum of one year nursing experience preferred.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Furnishes nursing services per plan of care in home settings.
• Prepares clinical and progress notes.
• Assists physician, IPC provider and RN in procedures.
• Maintains aseptic technique and prepares equipment.
• Educates patient and family on self-care per plan of care.
• Documents observations and treatments accurately.
• Participates in conferences and QM activities as assigned.
• Takes on-call duties, nights, weekends, and holidays as needed.
