Job summary

Jackson County Memorial Hospital Authority is seeking a Home Care Aide to provide personal care services under RN supervision, assisting patients with daily living activities, vital signs, and plan of care execution.

The role includes making regular home visits, performing household tasks to support care, documenting visit records accurately, and communicating effectively with patients, families, and Case Manager to promote independence and safety.

Responsibilities

• Assists patients with activities of daily living, including personal hygiene, exercises, ambulation, and meal preparation.
• Provides household duties such as cooking, light housework, linen changes, and laundry to support home care.
• Maintains basic positioning of bedbound patients and proper ROM per plan of care.
• Administers patient care as outlined in the plan of care and documents actions accurately.
• Supports patient independence and creates an appropriate setting for safety.
• Conducts home visits to patients as assigned and at the prescribed frequency.
• Assists with implementation of emergency actions per home care policy.
• Sets priorities and completes tasks accurately and efficiently.
• Takes vital signs as ordered and documents visit records legibly and timely.
• Assists patients with assistive devices as per the plan of care.
• Completes visit records accurately and legibly, using appropriate language, and returns promptly.
• Completes daily clinical logs accurately and on time.
• Communicates effectively with patients, families, significant others, and staff.
• Conveys factual information about patients to Case Manager or Director.
• Identifies and reports obvious patient problems or changes in condition to the RN case Manager.
• Demonstrates knowledge of growth and development principles across the lifespan.
• Contributes to accurate patient charges and prevents waste of hospital resources.
• Reports to work on time, adheres to schedule, and avoids excessive overtime.
• Follows JCMH policies for requested and/or emergency time off and maintains professional appearance and conduct.
• Supports necessary changes and maintains department harmony.
• Maintains regulatory requirements and completes annual educational requirements.
• Uses time card accurately and communicates the hospital mission, ethics, and goals as well as Home Care's mission and vision.

The Home Care Aide is responsible for providing personal care services according to written directions prepared by the R.N. and carried out under the supervision of the appropriate professional.

Demonstrates Competency in the Following Areas:

• Demonstrates ability to assist patients with activities of daily living, e.g. personal hygiene, exercises, ambulation, and meal preparation.

• Demonstrates correct Range of Motion on assigned patients per plan of care.
• Demonstrates ability to administer patient care accurately as per plan of care.
• Supports the patient and provides appropriate setting to encourage independence.
• Demonstrates ability to make home visits to patients as assigned at appropriate frequency.
• Demonstrates ability to assist with implementation of emergency actions as per home care policy.
• Demonstrates ability to appropriately set priorities and is able to carry out work accurately and efficiently.
• Demonstrates ability to take vital signs accurately as ordered on patients and documents legibly and accurately on visit records.
• Demonstrates ability to assist patients with assistive devices as per plan of care.
