Responsibilities

• Coordinates patient services under the direction of the team manager/designee.
• Prioritizes and coordinates daily patient schedules and requests additional services as needed.
• Ensures referrals for services are assigned in a timely fashion.
• Manages multiple phone calls and provides follow up to callers.
• Updates EMR per agency standards and attends team meetings.
