Job summary

Responsibilities

• Provide quality and safe home delivery of services under supervision of professional nursing staff.
• Assist with personal hygiene, dressing, bathing, shopping, and meals.
• Ensure essential environmental maintenance for patient safety at home.
• Perform basic health care tasks, monitor vital signs, and assist with medications.
• Coordinate preventive services as directed by the nurse.
• Communicate services and benefits to patients and families.
• Arrange transportation as needed for medical appointments.
• Collaborate with nurses, CNAs, and other in-home care professionals.

• Assists/provides patients with personal hygiene, dressing, bathing, and other daily living tasks, homemaking services, nutrition through meal preparation, teaching and emotional support.

• Reports any patient changes, concerns and incidents promptly.
• Completes documentation, visit forms, and time studies within time limits specified.
