RELIABLE SUPPORTIVE HOME CARE LLC is seeking a home care aide to provide compassionate, one-on-one care in patient homes. You will follow personalized care plans, maintain patient dignity, and document progress while building strong relationships with clients.

Qualifications

• Graduated from an accredited Home Health Aide program.
• High School Diploma or GED is required.
• One year of professional experience is preferred.
• A driver's license is required.
• CPR certification is required.

Responsibilities

• Assist patients with activities of daily living, such as moving in/out of bed, baths, wheelchairs, or vehicles.
• Care for patients by changing linens, doing laundry, cleaning the home, and assisting with personal care.
• Maintain records of patient care, condition, progress, or problems for supervisor review.
• Build relationships with patients by conversing or reading aloud to support mental alertness.
• Administer medications and immunizations as directed.
• Engage patients in exercises or other activities to promote health.

We are looking for a home care aide to join our team! You will be directly working with patients, following a one‑on‑one care plan in patient homes. You operate with a big heart, not only by providing personalized and attentive care but also by building genuine relationships with those you serve. You are detail‑oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior home health or hospice experience is a plus.
