In this role, you will work closely with patients, following personalized care plans and providing attentive support. Ideal candidates will possess excellent communication skills, be detail-oriented, and maintain a healthy environment for their patients. Prior experience in Home Health or Hospice is beneficial but not required. Competitive pay and flexible scheduling offered.

Qualifications

• Must pass a criminal background check.
• Pass a drug test.
• 2 Step TB or blood draw required.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, and wheelchairs.
• Care for patients by changing bed linens, doing laundry, cleaning, and personal care.
• Maintain records of patient care and discuss observations with the supervisor.
• Build relationships with patients by conversing or reading aloud.
• Engage patients in exercises or other activities.
• Light housekeeping tasks.
• Assist with bathing.
• Prepare meals.
• Provide medication reminders.

You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail‑oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus but not needed.
