Samash Holistic Home Care LLC is seeking a Home Care Aide to work one-on-one with patients in homes, following a care plan with heart and attention to detail. You will build genuine relationships, maintain patient records, and support health and well-being.

The role emphasizes strong verbal and written communication, daily activity assistance, and activities to keep patients engaged and emotionally supported. Prior Home Health or Hospice experience is a plus.

Qualifications

• Must have completed an accredited Home Health Aide program.
• Strong verbal and written communication skills.
• One year of professional experience preferred.
• Driver’s license required.
• CPR certification required.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Give medications and immunizations
• Engage patients in exercises or other activities
