Job summary

TRUST HOME CARE INC is seeking a Home Care Aide to work with patients in their homes, following a one-on-one care plan. You will be detail-oriented, organized, and focused on creating a healthy, supportive environment.

Strong communication skills are essential; prior home health or hospice experience is a plus. The role involves direct patient care, documentation, and coordinating with a supervisor to ensure quality service.

Qualifications

• Graduated from an accredited Home Health Aide program.
• High School Diploma or GED required.
• One year prior professional experience preferred.
• Driver’s license required.
• CPR certification required.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles.
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care.
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor.
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert.
• Give medications and immunizations.
• Engage patients in exercises or other activities.
