Job summary

Collage Home and Community Rehabilitation Partners in Chino, CA is seeking experienced Speech Language Pathologists to provide patient-centered care in patients' homes and communities. PRN positions available; case load will be in Chino and surrounding areas.

Qualifications

• Master's in Speech Pathology and state licensure required.
• ASHA CCC preferred; experience with neurologically related disorders is a plus.
• Excellent planning, organization, and communication with families and professionals required.

Responsibilities

• Evaluate and treat impairments in speech and language, executive functioning, cognitive needs, dysphagia, and augmentative communication in home settings.
• Deliver patient-centered therapy in patients' homes and communities.
• Collaborate with families, professionals, and the interdisciplinary team to implement integrated treatment plans.
