Qualifications

• Active Illinois RN or LPN license.

• Strong assessment and communication skills.
• Compassionate, dependable, self-motivated.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled, one-on-one nursing care in the patient’s home.
• Administer and manage medications.
• Monitor and document patient conditions.
• Perform treatments as ordered by the physician.
• Educate and support family members.
• Collaborate with the Phoenix clinical team for guidance and support.
