Job summary

Daymark Health is seeking an experienced Nurse Practitioner to deliver hands-on cancer care in patients’ homes and via virtual platforms, coordinating with our care team to implement the value-based model and expand services in Los Angeles.

You will roll up your sleeves as a player/coach, triage high-acuity patients, address symptom management, behavioral health, and social needs, while building trusted relationships with patients and their networks.

Qualifications

• Active NP or PA with at least 4 years of direct clinical care experience (oncology/home care/ palliative care/hospice).
• Proven ability to balance hands-on care with program development and process improvement.
• Strong communication, documentation, and teamwork skills; proficient in Slack/Zoom/Google Suite.

Responsibilities

• Provide direct patient care in home settings and via virtual platforms, including assessments and orders.
• Triage high-acuity patients and collaborate with oncology practices and clinical leadership.
• Support care continuum from active treatment to survivorship or end-of-life care.
• Build trusting relationships with patients and networks through empathy and listening.
• Collaborate with operations to refine and implement the Daymark care model.
