Job summary

Collage Home and Community Rehabilitation Partners in Katy, TX seeks an experienced Licensed Clinical Social Worker (LCSW) to provide patient-centered care in patients' homes and communities. This PRN role supports clients across the continuum of neurological and complex orthopedic needs in their own environments.

The ideal candidate will develop individualized treatment plans, collaborate with families and professionals, and contribute to durable recovery goals through evidence-based practices

Qualifications

• Completion of a Master's degree in Social Work and state licensure as an LCSW are required
• Excellent planning, organization, writing, and time management skills are required
• Ability to communicate with families, professionals, and co-workers is a must

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
