Collage Home and Community Rehabilitation Partners is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in Austin, TX and surrounding areas. PRN positions available for clinicians who will accept patients for the duration of their stay.

Our interdisciplinary teams create integrated treatment plans delivered in patients' homes and communities, with an evidence-based approach aimed at durable outcomes.

Qualifications

• Master of Social Work (MSW) degree required.
• State licensure as an LCSW is required.
• Excellent planning, organization, writing, and time management skills are required.

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
