Job summary

Collage Home and Community Rehabilitation Partners seeks experienced Occupational Therapists to provide patient-centered care in La Mesa, CA and surrounding communities. Therapists will perform home-based evaluations and plan functional treatment programs in clients' homes and communities.

The role emphasizes collaboration with interdisciplinary teams, evidence-based practice, and tailoring activities to address ADLs, cognitive needs, and occupational goals.

Qualifications

• Bachelor's degree in Occupational Therapy and state licensure required.
• Master's degree in Occupational Therapy preferred.
• Experience in neurologically related disorders preferred.
• Excellent planning, organization, writing and time management skills required.
• Ability to communicate with families, professionals, and co-workers.

Responsibilities

• Evaluation and treatment of impairments including upper extremity function, ADLs and IADLs, visual and perceptual skills, cognitive needs, adaptive equipment needs, assistive technology needs.
• Collaborate with interdisciplinary teams to create integrated treatment plans.
• Provide therapy in patients' homes and communities with a patient-centered approach.
