Qualifications

Responsibilities

• Perform initial, annual and ongoing risk assessments.
• Monitor cases and adjust service plans as needed.
• Provide backup support to other social workers and staff.
• Assist with relocation planning for clients.
• Obtain Medicaid eligibility and related documentation.
• Provide crises response and resource referrals.
• Resolve service provision issues with providers.
