Job summary

The Coordinating Center is seeking a Supports Planner to help individuals with complex medical needs live independently. You will coordinate services with clients, families, and providers, developing person-centered goals and ensuring access to community resources across Maryland.

As a home-based role, you will visit client homes and care sites, collaborating with nurses and social work consultants to resolve issues and support transitions back to the community.

Qualifications

• At least one year of case management experience for individuals with complex medical or behavioral health needs.
• Bachelor’s degree in human services or related field preferred.
• Experience with Medicaid and community resources strongly preferred.

Responsibilities

• Utilize assessments to advocate for client needs and develop person-centered plans.
• Coordinate supports, services, and goals with clients and care teams.
• Troubleshoot conflicts within the support team and address client crises.
• Perform quarterly in-home visits and monthly telephonic care coordination.
• Locate community resources and assist with transitions from medical facilities to home care.
• Document activities in electronic systems following billing guidelines.
• Manage own schedule to ensure timely completion of tasks.
