Job summary

Integrated Services for Behavioral Health is seeking a Community Behavioral Health Worker to join our team in Hocking County, Ohio. This role involves delivering home and community-based behavioral health services, assessing needs, and supporting individuals and families in self-management.

You will document activities, participate in program evaluation, and collaborate with partners to promote wellbeing. Requires 3 years of relevant experience or a Bachelor’s degree and a valid driver's license.

Qualifications

• Minimum three years of directly relevant experience or a Bachelor’s degree.
• High school diploma required.
• Demonstrated a high degree of cultural awareness.
• Experience with multi-need individuals and families.
• Broad knowledge of community service systems.
• Licensed and able to operate a motor vehicle with appropriate insurance.
• Willing to participate in and lead cross-systems team-building activities.
• Able to communicate effectively verbally and in writing.
• Proficient in operating in an Internet-based, automated office environment.

Responsibilities

• Assess strengths and needs of referred people, families, and communities.
• Provide home and community behavioral health services that are practical and collaborative.
• Meet people wherever they are with support for self-management of health and well-being.
• Meet billing productivity requirements established by Integrated Services for Behavioral Health.
• Maintain documentation, participate in program evaluation, attend team meetings, and learn about community resources.
• Collaborate in cross-systems training and expand knowledge of community services.
• Support growth in neighboring counties and other duties as assigned.
