Job summary

A leading healthcare provider in Brooklyn is searching for a compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to deliver acute care directly to patients at home. The position offers a competitive salary of $176,800 to $197,600 annually, along with a signing bonus after the first 90 days. Candidates must have a valid New York State license and at least two years of experience in high-acuity settings. This role requires flexibility for evenings, weekends, and holiday coverage. Benefits include unlimited vacation, medical plans, and a 401(k) plan.

Qualifications

• Minimum 2 years (3,600 hours) of experience in high-acuity settings.
• Experience in EMS with IV access and medication administration is a plus.
• Must hold a current, unrestricted New York State license.

Responsibilities

• Deliver problem-focused health assessments and interventions to patients at home.
• Order and analyze diagnostic tests including labs and EKGs.
• Coordinate care with referring clinicians and family education.
