Job summary

BronxWorks is seeking a Program Specialist (5087) in Bronx, NY for a full-time position. The role supports HIV/AIDS, Hepatitis C testing, outreach and client services across multiple sites, helping neighbors improve health and wellbeing.

Qualifications include a High School Diploma (BA preferred), preference for HIV/AIDS experience, and a valid NYS driver’s license. Travel across NYC boroughs is required; training is provided and accommodations are available upon request.

Qualifications

• Experience with people living with HIV/AIDS preferred; training provided.
• High School Diploma required; BA preferred.
• Food Protection Certificate preferred.
• Proficiency in modern business communications including PCs, email, voicemail, and copier equipment.
• Valid NYS Driver’s License is REQUIRED.

Responsibilities

• Provide direct services and prevention education to people living with HIV/AIDS and those at high risk for HIV/STIs and Hepatitis C.
• Administer HIV and HCV testing.
• Maintain familiarity with program resources on- and off-site.
• Act as program contact for client inquiries.
• Facilitate evidence-based interventions such as Healthy Conversations, Healthy Relationships and VOICES/VOCES.
• Provide education and information on PrEP and PEP; refer individuals as needed.
• Coordinate client services with other social services and providers via referrals.
• Drive agency medical van to testing and outreach locations.
• Fill the vehicle with gas when needed.
• Notify supervisor of vehicle maintenance issues.
• Refer and link clients to medical providers.
• Complete intake, risk assessment, service plan, referrals, advocacy and counseling.
• Handle intake and storage of food deliveries; pack and distribute food packages.
• Sort food, stock shelves, check expiration dates, discard expired items.
• Prepare and conduct educational workshops and presentations.
• Monitor and document client progress toward goals.
• Provide weekly and monthly reports; meet with supervisor monthly.
