Job summary

Allies for Health and Wellbeing seeks a Care Coordinator to identify individuals with HIV/AIDS not currently in treatment and connect them to medical, mental health, housing, and supportive services. The role promotes retention in care through harm reduction, motivational interviewing, and trauma-informed approaches.

Reporting to the Director of Outreach + Education, the CC collaborates with clinics, local AIDS service organizations, and government programs to improve health equity.

Qualifications

• Experience working with diverse communities and coordinating services from multiple providers.
• Knowledge of HIV care continuum and treatment access.
• Ability to engage clients and build trust to promote retention in care.

Responsibilities

• Identify and engage individuals with HIV/AIDS not in treatment.
• Link clients to medical, mental health, housing, transportation, and other services.
• Advocate to reduce barriers to care for clients of color.
• Accompany clients to appointments and coordinate care with an interdisciplinary team.
• Record client interactions in electronic health records and complete administrative paperwork.
