Job summary

Florida Department of Health is seeking an OPS Registered Nurse to serve as HUG-Me Nurse for Area 15, delivering family-centered care coordination to HIV-affected mothers and infants across multiple settings in southeast Florida.

The role involves travel, home and hospital visits, and collaboration with partners to ensure continuity and retention in care, with emphasis on prenatal to postnatal HIV services and staff education.

Qualifications

• Florida Registered Nurse license (FL RN) as per Florida Statutes.
• Current CPR certification.
• Valid Florida driver's license.
• Case management experience.

Responsibilities

• Assess client and family needs; develop, implement, monitor, and evaluate care coordination plans.
• Review records and labs; ensure follow-up, referrals, and linkage to services.
• Coordinate with health partners and providers for continuity of care.
• Document services; maintain client files and enter data into HMS, Careware, and program systems.
• Perform emergency, shelter, and outreach duties; work outside hours as required.
