BrightStar Care – Cuyahoga West seeks a compassionate Licensed Practical Nurse (LPN) with tracheostomy and ventilator care experience to provide specialized, one-on-one care in home and facility settings. Pediatric trach and vent experience is strongly preferred, serving Elyria, Lorain, and surrounding communities.

This role features weekday and weekend shifts, hands-on nursing, and meaningful patient relationships within a supportive clinical team.

Qualifications

• Active, unrestricted Ohio LPN license.
• Minimum one year nursing experience preferred.
• Tracheostomy and ventilator experience strongly preferred.
• Pediatric nursing experience preferred.
• Current BLS/CPR certification.
• TB clearance per agency requirements.
• Valid driver's license and reliable transportation.
• Ability to lift up to 50 pounds as applicable.
• Strong clinical judgment and documentation skills.
• Ability to follow care plans and work independently.

Responsibilities

• Provide skilled nursing care to pediatric and medically complex patients.
• Perform tracheostomy care and provide ventilator support according to the care plan and clinical protocols.
• Monitor patients' conditions, recognize changes, and respond appropriately.
• Administer medications and treatments as prescribed.
• Assist with activities of daily living and mobility as needed.
• Follow infection prevention, patient safety, and airway management protocols.
• Maintain accurate and timely clinical documentation.
• Communicate with physicians, families, and the clinical care team.
• Follow individualized care plans and participate in ongoing training and competency development.

Are you a compassionate and skilled Licensed Practical Nurse (LPN) with experience in tracheostomy and ventilator care? BrightStar Care is seeking dedicated nurses to provide specialized, one-on-one care to patients with complex medical needs in home and facility-based settings.

Schedule & Availability

• Weekday Shifts: Monday–Friday, 8:00 AM–4:00 PM
• Weekend Opportunities: Morning shifts, 8:00 AM–4:00 PM
• Service Areas: Elyria, Lorain, and surrounding communities
• Setting: In-home and facility-based patient care
