Who We Are

EMPACT- Suicide Prevention Center, part of the La Frontera family, is an industry‑leading non‑profit organization dedicated to providing valued community services and resources for behavioral health counseling, crisis intervention, and suicide prevention since 1987. We are an EEO/AA Employer.

What You’ll Do

The High Needs Case Manager works with children and families that require a high level of service provision, contact, and coordination of treatment out of our Tempe or Glendale location. This position provides direct service crisis intervention, support, transportation, and education to families and children, independently of, or in conjunction with, other in‑home client services. The role manages a caseload of 15 families to serve them at a high level of intensity.

Essential Responsibilities
  • Provide direct in‑home, in‑office, in‑school, and community services—crisis intervention, transportation, client stabilization, additional supervision, parent training, and support to keep a child in the home.
  • Facilitate, schedule, and provide follow‑up on family, youth, and adult team meetings (ACT or CFT). Complete written documentation and share with all team members. Engage natural supports and make referrals to collaborating partners as needed.
  • Deliver at least 40 hours of work per week, with a goal of 30 hours of direct client contact. Maintain clinical documentation standards and complete all tasks on the agreed timelines. Work a flexible schedule, including evenings or weekends when required.
  • Support services that meet the 12 Arizona Principles: collaboration, functional outcomes, unified assessment, comprehensive services, best practices, appropriate setting, timeliness, tailored services, stability, cultural respect, independence, and connection to natural supports.
  • Ensure continuity of care and coordinate communication with other departments and external stakeholders.
  • Develop and maintain effective working relationships with state, community mental health, and contracting organizations.
Qualifications
  • Bachelor’s Degree in Behavioral Health or related field preferred. Must meet BHT requirements.
  • Behavioral health experience required; experience working with children preferred.
  • Knowledge of the CFT process and Arizona 12 Principles.
  • Flexibility, problem‑solving skills, and creative approach to service delivery.
  • Fingerprint clearance card or eligibility to obtain one.
  • Valid AZ driver’s license with no major infractions in the past three years.
  • Reliable transportation.
  • Bi‑lingual (Spanish) preferred.
  • Minimum age 21 years at time of employment.
Additional Requirements and Responsibilities
  • Remain calm in crisis situations, work independently and cooperatively, recognize personal limitations, and relate positively and with empathy to all persons regardless of age, race, creed, gender, or sexual orientation.
  • Maintain professional conduct consistent with ethical practice and agency policies.
  • Maintain a professional demeanor and positive working relationships with internal and external team members, stakeholders, and community partners.
  • When driving for agency work, possess a valid Arizona driver license, liability insurance, and approval for liability coverage with EMPACT‑SPC’s liability insurance carrier. A 3‑year state driving record with no major infractions required.
  • Staff who provide direct client services must be at least 21; staff who provide support services must be at least 18 and have a high school diploma or equivalent.
  • Attend meetings, training, and agency activities outside of regularly scheduled hours upon request.
