A community-focused organization in Arizona is seeking a High Needs Case Manager to work with children and youth to develop service plans and goals. This full-time role involves providing social skills development and behavior management support, along with connecting clients to community resources. As a High Needs Case Manager, you will work directly with children and youth in the community to empower them to create their own service plans and goals. In this role, you will provide social skills development, behavior management support and education, and advocate for and connect clients to additional community resources. You will meet with clients and their families in their homes, the community, and the office to assess their needs, track progress, and adjust treatment plans as necessary.
