Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to support patients at home under an established care plan. This part-time role emphasizes personal care, safety, and clear communication with patients and families.

You will assist with daily activities, monitor vitals when ordered, and follow the plan of care while maintaining documentation and adherence to training and competency requirements.

Qualifications

• Must read and follow the patient care plan.
• Maintain CPR certification and comply with regulations.
• Complete Maxim competency programs as required.

Responsibilities

• Assist patients with baths, oral hygiene, dressing, toileting, feeding, and related tasks per care plan.
• Take and record temperatures, pulse, respiration and blood pressure when ordered.
• Assist with safe transfers and ambulation as directed.
• Provide exercises per the plan of care and document progress.
• Perform ROM and simple procedures as ordered, with competency.
• Handle patient laundry and safety needs; use equipment safely.
• Observe home safety practices and educate patients/families.
• Communicate clearly with client, family, and supervisor; report changes.

Job description

The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Responsibilities

• Responsible for assisting patients with and per plan of care, the following:

  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.
  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
  • Safe transfer and ambulation techniques
  • Exercise according to the plan of care
  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
  • Patient’s laundry
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
  • Observation and teaching of home safety practices
  • Recognition of an emergency situation and knowledge of emergency management procedures
• Must follow patients’ plan of care and complete all tasks assigned
• Communicates in a constant, clear, and effective way with the client, family/caregiver and office
• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
