Formación

• RN license in California is required.
• BLS certification required.
• 2 years of applicable RN experience within the last 5 years.
• At least 6 months of home health experience in a hospital-based or community-based home health agency.

Responsabilidades

• Provide the initial all-system nursing assessment prior to the provision of care.
• Initiate the plan of care and revisions including obtaining physician orders for changes.
• Provide care and services in accordance with the plan of care.
• Perform assessment/data collection in an ongoing systematic manner.
• Provide ongoing periodic all-system assessment of the patient and equipment needs.
• Initiate preventative and rehabilitative nursing procedures.
• Initiate timely discharges based on goals.
• Educate and instruct the patient, family, or other staff as required.
• Counsel the patient and family in meeting nursing and related needs.
• Supervise and teach LVNs and HHAs as required for patient care.

About This Role

What You'll Do

• Provide the initial all system nursing assessment prior to the provision of care.

Requirements

Education

• High School Diploma/GED
• Graduate from an approved school of professional nursing (AA, Diploma, or BSN)

Experience

• 2 years applicable registered nurse experience within the last 5 years
• At least 6 months of home health experience within a hospital-based or community-based home health agency

Licenses & Certifications

• Current RN License in California
• Driver's License (California)
• Basic Life Support certification required
