Qualifications

  • PGY2 completed in Oncology or Pediatrics preferred.

Responsibilities

  • Deliver direct patient care through pharmacotherapy management and medication optimization.
  • Educate patients and caregivers about medications, adverse effects, adherence, and safe handling.
  • Review and interpret medication orders for accuracy and appropriateness.
  • Collaborate with providers to support formulary compliance and access to specialty meds.
  • Lead and participate in medication safety, QA, and performance improvement initiatives.
  • Ensure compliance with policies, procedures, and regulatory requirements.
  • Supervise pharmacy operations and staff to ensure safe medication use.
  • Precept, mentor, and evaluate pharmacy learners and staff.
  • Deliver age-appropriate pharmaceutical care across care continuum.

Job description

Join our team of pediatric hematology and oncology pharmacists to care for our patients in the acute care and clinic spaces. Sign on bonus available.

Key Responsibilities:
  • Deliver direct patient care through evidence-based pharmacotherapy management and medication optimization.
  • Provide patient and caregiver education regarding medications, adverse effects, adherence, administration, and safe handling.
  • Review, verify, and interpret medication orders for accuracy and appropriateness.
  • Collaborate with healthcare providers to support formulary compliance and access to specialized medications.
  • Lead and participate in medication safety, quality assurance, and performance improvement initiatives.
  • Ensure compliance with all applicable policies, procedures, and regulatory requirements.
  • Supervise pharmacy operations and support personnel to ensure safe, effective medication-use practices.
  • Precept, mentor, and evaluate pharmacy learners and staff.
  • Deliver age-appropriate pharmaceutical care across the continuum of care.

Licensure, Registration, and/or Certification Required:

Licensed in the state(s) of practice or eligible for reciprocity required.

Education Required:

Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP.
Completion of a PGY2 in Oncology OR Pediatrics.

Experience Required:

No experience required.
Will consider: Doctor of Pharmacy with at least 5 years of clinical pediatric experience who has obtained their BCOP OR BCPPS.
