Tower Health’s Reading Hospital is seeking a Heart Failure Program Coordinator to coordinate and facilitate all components of the heart failure program and direct quality improvement efforts tied to accreditation standards.
The role focuses on delivering patient-centered, efficient care, integrating related disease programming, and fostering collaborative care with physicians and Reading Health System entities. Schedule is Monday–Friday, 7:00am–3:30pm.
