Job summary

Self Regional Healthcare is seeking a Heart Failure Navigator (RN) for the First Shift in Greenwood, SC. You will guide patients through cardiovascular services and support nursing staff to improve outcomes. 8:00 AM-5:00 PM, Monday-Friday.

You will educate nursing staff on treatment modalities and cardiovascular care, coordinate services within the cardiology team, and assist with Chest Pain accreditation processes and policy development using best practice guidelines.

Qualifications

• Active South Carolina registered nurse license.
• Minimum 5 years’ experience in cardiology.
• Knowledge of cardiovascular care and treatments.

Navigates patients through Cardiovascular services to provide care for these conditions or disease states. Assist nursing staff in providing quality
outcomes for Cardiovascular patients. Responsible for educating nursing staff with treatment modalities and care of the cardiovascular patients.
Assist with Chest Pain accreditation processes, protocols, and policy development through collaboration with providers and nursing using best
practice guidelines.

Job Qualifications:

• Licensed to practice as a Registered Nurse in South Carolina, 5 years’ experience in cardiology required.
• Routine nursing supplies, medications, cardio lab instruments, equipment and treatment devices specific
  to assigned patient population. Computer terminal and printout.
