UCSF Health is seeking a Nurse Coordinator for the Heart Failure Disease Management within the Advanced Heart Failure Comprehensive Care Center. The role coordinates care for patients being evaluated for general heart failure management and those moving into advanced therapies, bridging inpatient and outpatient services.
You will educate patients, coordinate testing and therapies, review referrals, and rotate through the Diuretic Clinic, performing IV access, blood draws, and IV medication
The Nurse Coordinator for the Heart Failure Disease Management portion of the Advanced Heart Failure Comprehensive Care Center coordinates care for our patients being evaluated for general heart failure management, as well as those who may be moved into advanced heart failure therapies workups. This role will coordinate between inpatient and outpatient departments, and manage patient panels including recommending diagnostic testing, lab testing, new and investigational heart failure therapies. This role regularly educates patients on heart failure disease, treatment, therapies, medication indications and the importance of adherence, and more. This role will interface between multiple UCSF departments, review incoming referrals, and coordinate outpatient care for patients with heart failure. This position will rotate through the Diuretic Clinic as needed with responsibilities including initiating IV access, blood draws, administration of IV medications, review of laboratory results, and follow‑up patient assessment.
