Job summary

UCSF Health is seeking a Nurse Coordinator for the Heart Failure Disease Management within the Advanced Heart Failure Comprehensive Care Center. The role coordinates care for patients being evaluated for general heart failure management and those moving into advanced therapies, bridging inpatient and outpatient services.

You will educate patients, coordinate testing and therapies, review referrals, and rotate through the Diuretic Clinic, performing IV access, blood draws, and IV medication

Qualifications

• 3 years’ ambulatory or acute care RN experience in Cardiovascular nursing including heart failure disease management.
• Strong MS Office computer skills (Word, Excel, PowerPoint), databases.
• Excellent organizational skills, ability to meet deadlines, and manage numerous concurrent tasks in high stress environments.
• Flexibility to orient and work at all UCSF Medical Center locations (ambulatory and acute care) for education and care coordination.

Responsibilities

• Coordinate care between inpatient and outpatient departments for heart failure patients.
• Manage patient panels and coordinate diagnostic testing, lab testing, and therapies.
• Educate patients on heart failure disease, treatments, medications, and adherence.
• Interface between UCSF departments, review referrals, and coordinate outpatient care for heart failure.

The Nurse Coordinator for the Heart Failure Disease Management portion of the Advanced Heart Failure Comprehensive Care Center coordinates care for our patients being evaluated for general heart failure management, as well as those who may be moved into advanced heart failure therapies workups. This role will coordinate between inpatient and outpatient departments, and manage patient panels including recommending diagnostic testing, lab testing, new and investigational heart failure therapies. This role regularly educates patients on heart failure disease, treatment, therapies, medication indications and the importance of adherence, and more. This role will interface between multiple UCSF departments, review incoming referrals, and coordinate outpatient care for patients with heart failure. This position will rotate through the Diuretic Clinic as needed with responsibilities including initiating IV access, blood draws, administration of IV medications, review of laboratory results, and follow‑up patient assessment.

Qualifications

REQUIRED QUALIFICATIONS

• 3 years’ ambulatory or acute care RN experience in Cardiovascular nursing including heart failure disease management.
• Strong MS Office computer skills (word, excel, power point), databases
• Excellent organizational skills, proficiency in meeting deadlines, ability to work independently and collaboratively with colleagues, prioritize workload and manage numerous concurrent tasks in high stress environments
• Demonstrated excellent written and verbal communication skills
• The flexibility to orient and work at all UCSF Medical Center locations, (ambulatory and acute care-for patients requiring education and care coordination while hospitalized)

PREFERRED QUALIFICATION

• Bachelor’s degree in nursing
• Master’s degree in nursing
• Working knowledge in assessing advanced heart failure, heart transplant
• Experience with managing and adjusting heart failure medications

REQUIRED LICENSES/CERTIFICATIONS

• Active Registered Nurse licensure in the State of California
• Active American Heart Association CPR certification
