Programmers.io is seeking a senior Epic Healthy Planet Analyst with strong population health and value-based care experience to lead configuration, optimization, and reporting initiatives.

The role emphasizes building Healthy Planet registries, risk models, care gap logic, and attribution workflows, delivering dashboards and analytics, and supporting HEDIS and Medicare Stars programs while ensuring data integrity across Epic, Clarity, Caboodle, and downstream systems.

Competenze

• Epic Healthy Planet Certification is required.
• Healthy Planet build and optimization experience.
• Registry build and maintenance experience.
• Population health workflow analysis experience.
• Quality reporting experience (HEDIS, eCQMs).

Seeking a senior Epic Healthy Planet Analyst with strong population health and value-based care experience.

Required:

• Epic Healthy Planet Certification
• Healthy Planet build and optimization experience
• Registry build and maintenance
• Population health workflow analysis
• Quality reporting experience (HEDIS, eCQMs)
• Care management and care gap management experience

Highly Preferred:

• Risk stratification and predictive analytics
• Patient attribution frameworks
• External data integration (claims, SDOH, HIE feeds)
• Medicare Stars and value-based care programs
• Clarity/Caboodle experience
• Care management tools, care plans, assessments, and outreach workflows

Responsibilities:
