Job summary

HomeWell Care Services is seeking an experienced, outgoing, and results-driven Healthcare Community Liaison to develop referral relationships and connect local families with high-quality non-medical in-home care. We need someone who enjoys being out in the community building relationships—not sitting behind a desk.

You will regularly visit hospitals, skilled nursing facilities, senior living communities, physician offices, hospice and home health organizations throughout South Bay San Diego.

Qualifications

• 2+ years of healthcare business development or referral sales preferred.
• Knowledge of the San Diego healthcare/senior-care community strongly preferred.
• Existing relationships with local healthcare referral sources are a major plus.

Responsibilities

• Build relationships with healthcare and senior-care referral partners.
• Conduct in-person visits to hospitals, SNFs, rehabilitation facilities, assisted living, memory care, physician offices, hospice and home health organizations.
• Develop relationships with discharge planners, case managers, social workers, nurses, senior-living professionals, and community leaders.
• Generate qualified private-pay home-care referrals.
• Schedule meetings, presentations, and lunch-and-learns.
• Follow up with referral sources and prospective clients.
• Coordinate with Intake and operations to convert referrals into starts of care.
• Meet weekly outreach and monthly business-development goals.
