Job summary

Woodhull Medical and Mental Health Center is seeking a hospital pharmacist to provide professional pharmacy services within a hospital setting, collaborating with clinicians to optimize therapy and patient safety.

The role includes filling prescriptions, reviewing orders, counseling patients, documenting interventions and ADRs, and using automated dispensing systems. Supervisory duties and involvement in quality programs may be part of the position.

Qualifications

Duties & Responsibilities
Purpose of Position:
This class of positions encompasses professional staff, supervisory, clinical and administrative work within a pharmacy department, with varying degrees of latitude for independent initiative and judgment. Health System Pharmacists adhere to Federal, State and local laws and other regulatory requirements regarding pharmacy and institutional practice. All personnel may be required to perform related work.
Examples Of Typical TasksUnder direction, with moderate latitude for the exercise of independent initiative and judgment, provides professional pharmacy services in a hospital, clinic or other health care setting. The following are typical tasks performed at Assignment Level I:

  • Fills prescriptions and patient orders, resolves discrepancies in patient therapy, and follows-up on problems.
  • Utilizes clinical data in evaluating physician orders to ensure appropriate therapy.
  • Provides and documents drug information and patient counseling.
  • Documents interventions, adverse drug reactions (ADRs), and medication errors.
  • Directs and utilizes personnel including technicians, aides and clerical staff.
  • Assists in the monthly inspection of the patient care units and the Pharmacy.
  • Maintains adequate medication supply in individual work area.
  • Participates in Pharmacy staff in-services and meetings relating to Systemwide and/or health care setting programs.
  • Operates and troubleshoots automated dispensing systems, as needed.
  • Advises prescribers on alternatives for non-formulary drugs.
  • Utilizes Outpatient and Inpatient computerized systems to enter and access patient care information.
  • Coordinates and analyzes statistics and/or clinical data.
  • Participates in quality management and utilization review programs.
  • May coordinate the work of a small group to accomplish a project or assignment.
  • May participate in the preparation and review of policies and procedures.
  • May assist in the development, implementation and execution of Pharmacy staff training curriculum.
  • In the absence of the supervisor, may assist in the supervision of a Pharmacy unit or function.
Minimum QualificationsPossession of a valid NY State license to practice pharmacy.
