This is a professional position responsible for recruiting eligible clients determined to be of high risk into PrEP (Pre-Exposure Prophylaxis) services. This person will be responsible for interviewing and entering client’s demographic, and services performed by Florida Department of Health in Miami Dade into Health Management System. Overall responsible for Perform STD/HIV/HEP C and pregnancy screenings in the mobile unit. Conducts counseling, testing and linkage (CTL) services in high incidence areas in the county. Position reports directly to the Prevention Manager.

Responsible for Tele-PrEP services. Interview clients face to face to determine interest in enrolling in PrEP. Determines purpose of the visit, answers customers’ questions, provides information and routes clients as per guidelines. Register clients in Health Management System (HMS). .. Interviews, verifies and records patient demographics and insurance/financial Prepares and witness consent forms and release of information. Ensures all services are documented into clinic portal in HMS inclusive of HIV 1-2 antibody Rapid Test, Lab HCV Rapid antibody test, HIV screening, Syphilis Rapid Test, Condoms, outreach services and collection of venous blood. -Complete consent forms for specified screenings (HIV/STD) 2- Responsible for completing accurate screening forms for HIV, STD Testing Lab and Hepatitis C screening form. 3. Educates clients and the public on sexual transmitted diseases & HIV/AIDS, including transmission and prevention to increase knowledge and awareness 4-Perform counseling and testing session following guidelines and protocols. 5- Screens client for HIV using rapid test kit and documents results in HMS. 6.Initiates and assists with Tele-PrEP session. 7. Based on clinician recommendations, proceed to guide client to mobile lab for further screenings. 8. Dispense & logs medication based on clinician recommendations. 9-Ask clients to complete client satisfaction survey. 10. Logs all referrals into PrEP log. 11. Responsible for updating Access database.

Participates in the operation of communicable and/or chronic disease control in mobile unit. Provides HIV education, prevention, HIV, STD’s, pregnancy and STD testing and linkage services within the selected target areas of Miami Dade County by participating in healthy teen expo, take control events, PSCI events and special events as assigned. Performs waived type laboratory tests such as Pregnancy test, Urine dipstick, rapid HIV, rapid HCV and other waived type tests per SOPs. Entering client data and results into HMS and into Logs ensures accuracy. For rapid HIV tests report results into CTLS, monitors test performance by comparing results with results from established State lab–based HIV testing algorithm, brings all discrepancies to manager attention, communicates results with testing coordinator and program manager. Places electronic orders in Emdeon/HMS for HIV confirmatory testing, ensuring completeness of 1628 forms, results documentation and confidentiality issues. Prepares and submits HIV reactive test log as well as 1628 forms with negative rapid results to the FLDOH HIV/AIDS Bureau on monthly basis. Assists with linking newly diagnosed clients to care or re-engaging HIV infected clients who have fallen out of care as needed. 7-Follow ups on preliminary reactive test in HMS to ensure that clients have been refer to appropriate services.

Oversee the data collection activities for the Tele-PrEP mobile services. Responsible for producing monthly reports monthly. Submits reports on activities performed to the Prevention Manager Facilitates counseling services through the Test Miami line. Ensure appropriate information is provided to the public. Information is not limited to addressing inquiries of COVID 19, hepatitis, PrEP, HIV, STD screenings and transferring callers to appropriate department.

Collaborates with DIS (Disease Intervention Specialist) for linkage, and re-linkage to prevention services, STD screening, and follow up. Maintains communication with PrEP Navigators, and STD for successful coordination of services. Assist in documenting program activities and report outcome in PrEP data collection tool. Assists and participates in Telehealth services for PrEP and molecular cluster investigations.

Perform other related duties as assigned.

Qualifications:

Minimum – Valid driver’s license, access to a private automobile to perform work related duties, experience conducting outreach activities, experience as a phlebotomist.

Preferred - The incumbent in this position is preferred to be bilingual in English and Spanish or English and Creole.
