Job summary

Qualifications

• Educates patients on screening results, preventative health maintenance, smoking cessation and NRT.
• Reviews radiology reports for incidental findings and makes appropriate referrals.
• Documents accurate documentation of each patient encounter and ensures compliance with regulations.
• Performs patient outreach regarding required follow-up testing and screening compliance.
• Schedules and coordinates patient exams and follows up with healthcare professionals and patients regarding results.
• Collaborates with physicians, hospital administration and staff to improve patient continuum of care.

Responsibilities

• Educates patients on screening results and health maintenance.

• Documents patient encounters and maintains confidential records.
• Outreaches to patients for follow-up testing and screening compliance.
