Job summary

The Legacy Link, Oakwood, GA, is seeking a Health Programs Registered Nurse to perform MDS-HC assessments and coordinate care for the Elderly and Disabled Waiver program across a broad geographic area. Travel is involved as part of the role.

You will work within an interdisciplinary, person-centered team to support clients, collaborate with Care Coordinators, and ensure compliance with program manuals and HIPAA requirements.

Qualifications

  • Graduate of an accredited nursing program.
  • Two years’ RN experience in geriatric, community health, long-term care, or chronic diseases of adults.
  • Current Georgia RN license.

Responsibilities

  • Meets standard of promptness for all admission referrals.
  • Meets standard of promptness for all annual and requested assessments.
  • Determines eligibility for the program by utilizing the MDS-HC and the intermediate level of care criteria.
  • Explains to the client/representative all aspects of the program and obtains member signature on all necessary forms.
  • Uses a comprehensive approach to discuss and clarify the client’s needs during case conferences
  • Collaborates with Care Coordinator in the development of the Comprehensive Care Plan to maintain or improve the health/functional status of the client.
  • Supports Care Coordinators on his/her team in dealing with clinical concerns
  • Maintains knowledge of the Elderly and Disabled Waiver Program Manual and Provider Service Manuals
  • Attends hearings as requested, providing data and member records required by the hearing officer
  • Acts as a liaison between the client, family, provider and physician to solve problems, complaints, non-compliance, etc.
  • Reports suspected abuse, neglect, exploitation to APS and inputs data into the DCH Incident Reporting System
  • Maintains and adheres to an updated Elderly and Disabled Waiver Program Manual
  • Attends monthly staff meetings
  • Adheres to HIPAA guidelines at all times to ensure privacy of client records

ACCOUNTABILITY: Reports to Health Programs Nursing Manager

STATUS: Exempt, Full-time- TBD schedule

JOB SUMMARY

The Health Programs Registered Nurse performs work of moderate difficulty by executing the MDS-HC for determination of services offered within the Elderly and Disabled Waiver Program. She/he functions as a member of an interdisciplinary, person-centered team and serves large geographic areas which may include parts of one large county and/or many small counties. Travel involved.

DUTIES AND RESPONSIBILITIES
Essential Functions
  • Meets standard of promptness for all admission referrals
  • Meets standard of promptness for all annual and requested assessments
  • Determines eligibility for the program by utilizing the MDS-HC and the intermediate level of care criteria.
  • Explains to the client/representative all aspects of the program and obtains member signature on all necessary forms.

Requirements

EDUCATION, EXPERIENCE AND SKILL REQUIREMENTS

Education:

  • Graduate of an accredited Nursing School or Program

Knowledge and Experience:

Two years’ experience as a Registered Nurse in one of the following areas:

  • Geriatric nursing
  • Community health
  • Long term care
  • Chronic diseases of adults

Skills:

  • Ability to communicate and coordinate effectively with clients, family members, service providers, other agency staff, and care coordination staff.
  • Proficient computer skills with ability to learn agency systems.
  • Well organized and detail oriented.
  • Demonstrates knowledge of health care and human services programs.
  • Knowledge of community organization and service system development.
  • Knowledge and understanding of the CCSP program.

Licensure and Certification

  • Valid State Driver’s License
  • Current valid Georgia RN License

  • May be required to walk on uneven terrain to get to client homes

  • Regularly required to read documents and write neatly, legibly, and transcribe accurate information and numbers/values
  • Employee continually engages in activities that require talking and hearing
  • Frequent: standing, sitting, walking, repetitive use of hands, simple grasping and fine hand manipulation, pushing and pulling hand. Occasionally required to lift or reach above shoulder level
  • The employee is occasionally required to stand, climb or balance; stoop, kneel, crouch or crawl.
  • The employee must occasionally lift and /or move up to 25 pounds.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
  • The noise level in the environment is moderate as phone conversations with the hard of hearing occur daily.
  • Temperature ranges from normal indoor climate-controlled environment in buildings or vehicles, various outdoor conditions and temperature extremes encountered during off-site travel, and unpredictable indoor environmental conditions encountered in client homes or other off-site locations

Generally, the job requires 50% sitting, 25% walking, and 25% standing.
