TrinityHealth Fresno is seeking a full-time Health Promotion/Chronic Care Specialist to advise clients on healthy lifestyles and manage chronic conditions. You will work with doctors, pharmacists, and staff to deliver education and coordinate care, including group seminars and health education materials.
Responsibilities include counseling on Medicare STARS, HEDIS, and HCC metrics, using motivational interviewing to identify barriers and develop client-specific wellness plans.

Description: Reporting to the Director CVHP Operations, CVHP Operations, this position is responsible for working closely with health plan member clients, physicians, pharmacists, and physician office staff, via phone, internet, in person and/or through group seminars, and distribution of health education materials to promote wellness. The incumbent is responsible for advising and counseling on the important requirements for a healthy lifestyle, working with clients to identify health risk factors and manage chronic illness, and motivate individuals to realize their optimum health potential. This position also advises and counsels on the important requirements of Medicare STARS, Healthcare Effectiveness Data and Information Set (HEDIS), and Hierarchical Condition Categories (HCC) with provider staff to identify health risk factors and gaps in quality of care. The incumbent will interface with clients using motivational interviewing and reflective listening skills in order to develop an understand of barriers to meeting health related goals, develop a client specific wellness plan, set goals for health improvement, and monitor goal attainment. The incumbent will coordinate services with other health-related departments and organizations for optimal patient care and satisfactory outcomes.
Pay Range $29.25 - $40.95
