Job summary

American Health Plans, a division of American Health Partners, is seeking an implementation specialist to collaborate with nursing home operators to roll out SNF engagement initiatives and Model of Care programs.

The role focuses on managing cross-functional projects, maintaining facility partnerships, and delivering on enrollment and performance metrics while ensuring compliance with program requirements.

Qualifications

• 3+ years of experience in health care / managed care organization with demonstrated track record of increasing responsibility and accountability to include leading and facilitating teams.
• Experience with value-based and risk-based negotiating arrangements.
• Demonstrated success building relationships with external executives and stakeholders; possesses a strong ability to persuade.

Responsibilities

• Lead and facilitate cross-functional implementation projects across teams to meet schedule and budget.
• Build and sustain facility-level relationships with partners to support plan goals.
• Communicate KPIs and progress to stakeholders; create mitigation plans as needed.
