Responsibilities

Job description

JOB SUMMARY

The Health Navigator assists individuals in reducing and eliminating barriers to healthcare and Social Determinants of Health. Barriers including access to care, insurance or lack thereof, poor health literacy, transportation, childcare, public assistance, housing, benefits enrollment, and more. Navigators build effective working relationships with their clients, helping to support, educate and assist clients to navigate the complex healthcare system. The Navigator works to identify their client's physical, emotional, and cultural needs and helps them access appropriate resources to meet these needs. The Navigator works within the concept of Social Determinants of Health in order to address needs that impact health.

SUPERVISION RECEIVED

Receives direct supervision from the Public Health Director.

ESSENTIAL FUNCTIONS

  • Develop strong relationships with clients to asst them with their care.
  • Work collaboratively with other staff members to ensure clients receive exceptional service and care.
  • Maintain a strong network with local and regional partner agencies and represent the department at our monthly local Multi-Tier Care Coordinator meeting
  • Educate clients about their healthcare, insurance options and benefits.
  • Take Connect 4 Health Assistor training in order to be an Insurance Assistor for the department.
  • Connect clients with community resources as needed.
  • Work with clients that have high system utilization to access care appropriately.
  • Maintain a record of client interactions.
  • May support clients in scheduling clinical and Medicare visits to support client needs.
  • Receive inbound client calls as well as partner agency referrals, and making outbound calls to clients and sending referrals to partner agencies
  • May perform home visits and transportation as meets the clients' needs
  • Demonstrate compassion for individual needs, attention to detail, and collaboration with fellow staff members.
  • Be available to work in remote locations such as Arboles or at partner agency office locations if requested
  • Work with Women's Cancer Screening and Detection team to provide care coordination and assess for insurance availability.
  • Other tasks as assigned such as minimal administrative work with a partner organization called Pagosa Outreach Connection and front desk coverage when needed
  • Marketing campaigns for Connect for Health Colorado
  • Annual presentation to the Board of Health regarding program updates and outcomes
  • Quarterly reports that reflect program updates and quarterly outcomes
MINIMUM QUALIFICATIONS

Any combination of experience and training which provides the applicant with the level of required knowledge and abilities will be considered.

  • High school diploma or equivalent required.
  • Minimum 3 years of experience assisting clients and families or other related healthcare/community experience required.
  • Knowledge of state social service agencies and community resources is preferred.
  • An Associate degree, in a related field, may substitute for the experience requirements on a year-for-year basis.
  • Must possess a valid driver's license.
  • Bilingual (Spanish) is desirable.
  • Registry on the Health Navigator Registry desirable; completion of the Community Health Worker credential within the first year of employment
KNOWLEDGE, SKILLS, AND ABILITIES
  • Knowledge of business English, spelling and math.
  • Knowledge of standard office practices and procedures.
  • Knowledge of socio-economic conditions of the community and social agencies.
  • Knowledge of public health and/or medical settings preferred.
  • Ability to operate standard office equipment, including a computer.
  • Ability to prepare and maintain accurate and neat records.
  • Ability to provide participant services to assigned caseload and complete paperwork within time limits.
  • Ability to establish and maintain effective working relationships.
  • Ability to work with minimal supervision; ability to work as a team member.
